If you are a recurring therapy client, please check the box authorizing recurring payments on the second screen of the payment form. Appointments cancelled with less than 48 hours advance notice will result in the full fee. If you have any questions, or wish to discontinue services at any time, please contact us by filling out the form at the bottom of this page.
